Pre Admission
Your booking will be made through your doctor’s surgery. Once you have agreed to the procedure be sure to discuss the following with your doctor before signing the consent form:
- What is the reason for admission, and what procedures will take place?
- How long will you be in hospital?
- When (date and time) will you be admitted to the hospital
- Should you have any tests or xrays beforehand?
- Should you fast before you are admitted?
- Do you need to alter your medications?
- What fees will be charged?
- What plans should you make following discharge (eg if you will be restricted in lifting or driving – How you will manage housework or shopping?)
- It may also be advisable to contact your general practitioner if you are a surgical patient to advise them of your impending admission date.

Pre Admission Information
Our Pre Admission Office is open from 9 am to 4.30pm Monday to Friday, and it is preferable that you attend in person to complete or deliver the necessary paperwork. If you are unable to do so, please telephone our Pre Admission coordinator with your details on (02) 4389 9465.
What to bring for Pre Admission
- Letter from your doctor and contact details of your GP
- Medicare card
- Health Fund membership book or card
- Department of Veterans’ Affairs card, any Pension Entitlement card, any Pharmaceutical Entitlement card
If you are privately insured, self insured or partially insured please read the Accounts section.
If you have acquired any illness (including colds or chest infections) since consulting with your doctor, please notify them prior to your admission to hospital.
